CLAIMS INFORMATION

If you, your business, or one of your employees experiences an accident involving property damage
or personal injury and needs to know what to do, please follow the proceedures as indicated below:
AUTO ACCIDENTS 1.
Stop at once 2. Seek Medical Attention for anyone who is hurt 3. Call the police 4. DO NOT ADMIT FAULT 5. Discuss
the specifics of the accident only with the police and your insurance agency 6. Exchange information with the other people
in the accident; obtain names, addresses, insurance policy number, insurance company and PHONE NUMBERS 7. If witnesses
stop, obtain their names and phone numbers 8. Contact the Morrow Insurance Agency, Inc. claim department at (574) 936-2400
PROPERTY
DAMAGE 1. Secure the premises to prevent further damage; you may need to call a general contractor to assist you 2.
Call the Morrow Insurance Agency, Inc. claim office at (574) 936-2400 and be ready to describe the cause of the property damage;
the address of the damaged property, and the date the claim occurred
EMPLOYEE INJURIES 1. Require
the employee to complete an employee injury report describing the date of the injury, the accident, the body parts injured
and the location of the accident 2. Complete the first report of injury form and send promptly to our claim department

Investigate the accident to identify and implement corrective action to prevent a similar type of accident
LIABILITY
CLAIMS AND LAWSUITS 1. If you receive a letter, summons or claim notice from a third party alleging that you
damaged or harmed them in anyway, send the documents to Morrow Insurance Agency, Inc. immediately 2. If the document is
a summons from the court, you have a limited number of days to make a court appearance and the insurance company will do this
for you, if we receive the documents in a timely fashion 3. If a visitor is injured at your property, note the event time,
date, visitor name and description of the accident and injuries. Take pictures of the accident scene, if possible, and contact
us to report the potential claim.

Some of our insurance companies are now offering you the opportunity to report your claim directly 24
hours a day/7 days a week. In addition, you may be able to check on the status of your claim, as well. Please
